Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
  • Do I have mental health insurance benefits?
  • What is my deductible, and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?

Kenneth and I are both in-network providers with many managed care networks and also some Employee Assistance Programs (EAP) companies. Our practice offers a specialized customer service feature for obtaining eligibility and a quote of benefit from a member's insurance provider. We do this so we understand what the plan allows, what types of limits or exclusions there may be, and whether the services are subject to a deductible, co-insurance, co-pay, office visit, specialist fee, or if there are out-of-pocket maximums. This reduces billing errors, and we take pride in our ability to work with a number of health plans. In the case of the special benefit of Employee Assistance Programs (EAP), our office needs to receive an authorization number, or the member needs to have an authorization number before services are rendered.

As of 10/1/2016, a late cancellation or no-show fee will apply. Our office will bill you for the full charge of the session. We respectfully request 24-hour advance notice in the event an appointment needs to be changed. Your appointment time is exclusively reserved for you. We can not help you if you are not here.

If you are unable to locate Kenneth Gleaves or Nancy Imler-Gleaves in the directory, call our office at 708.422.2885 and let us know. We will research the issue.

Choosing an in-network provider allows the fullest benefit of your plan. Out-of-network benefits may be available for providers that are considered out of network with your plan. Or, your plan may not have any out-of-network benefits.
